We collect personal information you provide directly (e.g., when you complete intake forms, contact us, or book appointments through JaneApp). This may include your name, date of birth, contact details, health history, and payment information.
Use of Personal Information
Your information is used to provide therapy services, manage appointments, maintain records, process payments, and communicate with you.
Storage and Security
We use JaneApp, a PHIPA-compliant platform, to securely manage appointment scheduling, charting, and communication. Your information is encrypted and protected against unauthorized access.
Disclosure of Information
We will not disclose your personal health information without consent, except as required by law (e.g., risk of harm to self/others, child protection laws, or court orders).
